Important Instructions
1. Only online submission of forms facility should be used.
2. Once the form is submitted online, a print out of that should be taken.
3. The printout will contain 4 pages including two pages of pay in slip in triplicate one of J&k Bank another of SBI Bank.
4. The pay in slip in triplicate should be used for submission of Fee in any of two Banks.
5. Fee of Rs. 750/= should be deposited in any of the branches of the J&K Bank or State Bank of India Ltd.
6. Application form along relevant documents should reach office of the DCDC by or before 26 / 07 / 2012.
